2. Schedule an Intake Appointment with our Program Coordinator to determine your weight-loss surgery and insurance options. Give us a call at 425-778-2220, or toll free at 1-877-LIV-AGAIN (1-877-543-2424) to setup an appointment.

3. Bring the following to your Intake Interview appointment:
a - A copy of your benefit booklet and/or any information regarding your health insurance coverage.
b - A completed Health Questionnaire. Click here to download a Health Questionnaire – please print the form, and answer all questions as completely as possible.
